NEW PATIENT FORMS

Your First Visit

During your first visit to our practice, our friendly team members will greet you and help you complete your new patient paperwork if you haven’t already done so. We’ll then promptly escort you back to have your consultation with one of our chiropractors. You’ll have the opportunity to explain what’s brought you in and ask any questions that you may have.

Your chiropractor will then perform a quick exam and take X-rays if necessary. After, you’ll schedule your follow up appointment to allow us ample time to review your report of findings, which will be discussed on your second visit.

Your Second Visit

Your chiropractor will go over your report of findings in full detail. We want you to always feel as comfortable as possible with us, so please ask us any questions that you have. We’ll also make our recommendations for your treatment that will be unique to your specific needs and goals in health. If you’re in agreement, we can begin your care on this same visit.

After completing your initial care plan and when you’re feeling better, visits to our practice are entirely up to you. Many patients choose to continue care with us once they realize how beneficial adjustments are for their everyday health, but we will never force you to do something you don’t want to do. We will, of course, be here for your needs in whichever capacity you’d like.
